This paper (working document) illustrates strategies aimed at engaging patients, their families, and carers to be an active part of health disease management and treatment, guiding them to make informed choices. Furthermore, it outlines strategies aimed at empowering populations to adopt responsible health lifestyles and act as protagonists in influencing determinants of health in a human-rights based approach to health. This project aimed to develop a Diversity Conceptual Model to support the aged care sector to identify diversity characteristics and associated benefits and disadvantages in order to consider greater equity in policy and practice.

Effects of community health interventions on under-5 mortality in rural Guinea-Bissau (EPICS): a cluster-randomised controlled trial

Evidence suggests that community-based interventions that promote improved home-based practices and care-seeking behaviour can have a large impact on maternal and child mortality in regions where rates are high.
