Scotland´s House of Care programme aims to facilitate a fundamental shift in the relationship between person and professional, so that the person is in the driving seat of their health and social care, with self management at the heart of it.

This approach supports and enables people to articulate their own needs and decide on their own priorities, through a process of joint decision making, goal setting and action planning.

For most children, the GP is their main point of contact with the health servivce. While children make up nearlyy one-fifth of the population in England, they are estimated to account for two-fifths of a typical GP´s workload. Despite this, the Royal College of Paediatrics and Child Health estimate that in many parts of the country, between 40 and 50 per cent of GPs have a had little or no formal paediatric training. Policy makers in many countries are encouraging the development of integrated care strategies and the development of new models of integrated care. These new models require changes at a clinical or service level, organizational level and system level with strong leadership necessary at all three levels. As a result of the changes, the health system is supporting more people in their homes and communities and has moderated demand for hospital care, particularly among older people. Change was achieved through developing a number of new delivery models, which involve better integration of care across organisational and service boundaries, increased investment in community-based services, and strengthening primary care. How to Integrate HIV and Sexual and Reproductive Health Services in Namibia, the Epako Clinic Case Study

During the past two decades, HIV and Sexual and Reproductive Health services in Namibia have been provided in silos, with high fragmentation. As a consequence of this, quality and efficiency of services in Primary Health Care has been compromised. 

The authors conducted an operational research (observational pre-post study) in a public health facility in Namibia. A health facility assessment was conducted before and after the integration of health services.
