Healthcare system reforms are pushing beyond primary care to more holistic, integrated models of community based primary health care (CBPHC) to better meet the needs of the population. The Vanguard of Community-based Integrated Care in Japan: The Effect of a Rural Town on National Policy

Japan has the largest percentage of elderly people in the world. In 2012 the government implemented a community-based integrated care system which provides seamless community helahtcare resources for elderly people with chronic diseases and disabilities. This paper explains the establishment and outcomes of Japan´s first community-based integrated care system in the rural town because the history of Mitsugi could inform policy making in other countries with aging populations.

Case Management for Patients with Complex Multimorbidity: Development and Validation of a Coordinated Intervention between Primary and Hospital Care

In the past few years, healthcare systems have been facing a growing demand related to the high prevalence of chronic diseases. Case management programs have emerged as an integrated care approach for the management of chronic desease. Nevertheless, there is little scientific evidence on the impact of using case management program for patients with complex multimorbidity regarding hospital resource utilisation. 

This study tested wheter improved continuity of care resulting from the integration of community-based and hospital services reduced the use of hospital resources amongst patientes with complex multimorbidity.

The Effective Use of Psychiatric Mental Health Nurses in Integrated Care: Policy Implications for Increasing Quality and Access to Care

In the last ten years primary care providers have been encouraged to implement integrated models of care where individuals' medical and mental health needs are addressed holistically. Many integrated models use Psychiatric Mental Health (PMH) nurses as case managers and select exemplars use PMH Advanced Practice Nurses (APNs) as providers. The role of informatics in patient-centered care and personalized medicine

This article covers the critical role that laboratory information systems, electronic medical records, and digital imaging plays in patient-centered personalized medicine. The value of integrated diagnostic reports, clinical decision support, and the use of whole-slide imaging to better evaluate cytology samples destined for molecular testing is discussed. This article also highlights the barriers to the widespread adoption ot these disruptive technologies due to regulatory obstacles, limited commercial solutions, poor interoperability, andlack of standardization.

Development, implementation and evaluation of a nurse led integrated, person-centred care with long term conditions

This proposal outlines a conceptual plan for innovative, integrated care designed for people living with long term condicions. This conceptual plan delivers a partnership between the health system, the person with long-term conditions (chornic), their family, and the community. The partnership aims to support people at home with access to effective treatment, consistent with the New Zealand Government Health Strategy. This concept of people-owned care is provided by nurses with advance practice skills, who co-ordinate care across services, locations and multiple long-term conditions.
