Patient-Centered Methods for Designing and Developing Health Information Communication Technologies: A Systematic Review

Chronic disease management and maintaining healthy behaviors to prevent disease are important lifelong considerations. Adherence to prescribed management and behaviors often falls short of physician recommendations, which can result in negative health outcomes. Information communication technologies (ICTs) offer an approach to combat this issue. However, uptake and sustainability of ICTs have mixed results. One reason could be that technologies are often created without an understanding of the complexities of patient needs. Therefore, the intent of this study is to explore the current landscape of patient-centered design and development of health ICTs through a systematic review.

Mobile Health for Ageing (mAgeing)

Health information, advice, and reminders delivered through mobile phones can encourage healthy behaviors and help older people to improve and maintain their intrinsic capacity. The WHO mobile health for Ageing (mAgeing) programme has been developed as one of the tools to support the implementation of WHO guidelines on community-level interventions to manage declines in intrinsic capacity – also known as the ICOPE Guidelines. The impact of a comprehensive electronic patient portal on the health service use: an interrupted time-series analysis

EPPs hold promise for reducing hospital readmissions. Certain patient populations with chronic conditions may differentially benefit from portal use depending on their needs for communication with their providers.
However, there is little empirical research on the potential benefit that electronic patient portals (EPP) can have on the care quality and health outcomes of diverse multi-ethnic international populations. The purpose of this study is to determine the extent to which an EPP was associated with improvements in health service use.

A compendium of tools and resources for improving the quality of health services

In an attempt to consolidate the work of the WHO Department of Service Delivery and Safety (SDS) on quality improvement, a SDS cross-cut team has produced a compendium of tools and resources on quality improvement, developed within the SDS, that are applicable for country support. Community-Based Social Innovations

This research defined CBSIs as community initiatives that seek to: a) empower older people to improve their self-efficacy in caring for themselves and their peers; b) maintain their well-being and; c) promote social cohesion and inclusiveness. Health systems across both developed and developing regions struggle to meet the diverse and complex needs of increasing number of older persons. This study hypothesized that community based social innovations (CBSIs) can improve the health and well-being of older people through community-based care. 

Project INTEGRATE: Developing a Framework to Guide Design, Implementation and Evaluation of People-centred Integrated Care Processes

People-centred integrated care is an acknowledged approach to improve the quality and effectiveness of health systems in delivering care around people’s needs and preferences. Nevertheless, more guidance on how to effectively design, implement and evaluate the care process of people-centred integrated care services is needed. Under Project INTEGRATE, a framework was developed to guide managers in the assessment, transformation and delivery of these health service innovations.

Why integrated care already owns the future

Integrated care has certainly arrived in the primary care clinic. Evidence and momentum began building 20 years ago with the demonstrated effectiveness of collaborative care programs for depression and anxiety disorders in primary care. Since then, various forms of integrated care have been repeatedly proven effective across a range of care settings, patient populations, languages and cultures.

Integrated care models are well adapted to thrive during the Fourth Industrial Revolution in health care. Because integrated care has embraced flexibility and disruption, integrated care models will succeed—or even dominate—in a future that will demand flexibility.

This directory of person-centred care organisations around the world was produced as part of an international environment scan of the field of person-centred care undertaken by Health Policy Partnership.
