COVID-19 case fatality rates suggest a notable increase in risk after age 60, with progressively worse outcomes for those over 70 and over 80 years. As the national rhetoric focuses on supporting the delivery of population health and integrated care, different models of care and new ways of working are being implemented across England. While integrated care systems (ICSs) are emerging to lead on planning and commissioning care for their populations, providers are also working together to deliver integrated care locally. 

This two-day congress provides an established forum for health and care professionals to come together and learn from successful adoptions and practical implementations of digital health and care. The aim of the event is to share learning and encourage adoption, so your project should be active, showing digital technology in practice under one of the project themes above.

Project themes

This conference is for care home providers, health care providers, and commissioners who are thinking through how to improve the quality of life and health care for people living in care homes. It will share lessons from the six vanguards for enhanced health in care homes, and other local examples of good practice, to enable attendees to learn from initiatives that have worked well and can be replicated across the country.

The workshop is a knowledge exchange activity organised as part of the inter-regional collaboration on digital health and care between TicSalut in Catalonia and NHS 24 in Scotland.

Catalonia developed one of the first regional mHealth strategies in Europe and has many lessons to share with Scottish stakeholders relating to the development, accreditation and implementation of mHealth apps and services.
