Improving Care for People with Long Term Conditions

This report, published by the Royal Pharmaceutical Society in Scotland, argues that pharmacist led care of people with long term conditions could deliver better results and be more cost effective. It focuses on the role of the pharmacist as part of a multidisciplinary approach to tackling the challenges facing the NHS in providing the highest quality care and support for people with long term conditions.

Scotland´s House of Care programme aims to facilitate a fundamental shift in the relationship between person and professional, so that the person is in the driving seat of their health and social care, with self management at the heart of it.

This approach supports and enables people to articulate their own needs and decide on their own priorities, through a process of joint decision making, goal setting and action planning. 

Building Competencies for Integrated Care: Defining the Landscape

As the discussion in integrated care moves from "what needs to be done?" to "how to do we achieve sustainable change?" one of the key questions is "how do we get people to change?". Research funders in Canada and abroad have made substantial investments in supporting collaborative research approaches to generating and translating knowledge as it is believed to increase knowledge use. Canadian health research funders have advocated for the use of integrated knowledge translation (IKT) in health research, however, there is limited research around how IKT compares to other collaborative research approaches. The aim of this study was to better understand how IKT compares with engaged scholarship, Mode 2 research, co-production and participatory research by identifying the differences and similarities among them in order to provide conceptual clarity and reduce researcher and knowledge user confusion about these common approaches.

Person-Centered Healthcare Practice in a Pandemic Context: An Exploration of People's Experience of Seeking Healthcare Support

The recent COVID-19 pandemic increased pressure upon healthcare resources resulting in compromised health services. Enforced national lockdown led to people being unable to access essential services in addition to limiting contact with social support networks. The novel coronavirus, and subsequent condition known as long covid were not well-understood and clinicians were not supported by existing guidelines or pathways. Our study explored people's experiences of healthcare during this period with a person-centered “lens.”
