Digital Health and Care Congress 2016. Enabling patient-centred care through information and technology

The congress will feature presentations and case studies from: NHS trusts; clinical commissioning groups; local authorities; voluntary sector organisations; universities and private sector providers, who will present innovative projects that are using technology and data to deliver high-quality sustainable health and care services.

The Digital House of Care: Information solutions for integrated care

This paper draws on the recent experience of achieving large-scale data integration accross the boundaries of health and social care, to help plan and commission services more effectively. The purpose is to describe the development of a digital tool in an English county stiving towards a vision of integrated information that is used to underpin and increasingly integrated future of health and social care delivery.

HealthBuddy: a new chatbot to engage with communities in Europe and central Asia on COVID-19

HealthBuddy, a multilingual interactive chatbot, is the newest tool to be launched as a resource for countries in Europe and central Asia in response to the COVID-19 pandemic. The chatbot, developed by WHO/Europe and the United Nations Children’s Fund (UNICEF) Europe and Central Asia Regional Office (ECARO), will help countries in the region to access accurate information, and counter misinformation surrounding the virus.

Applying Complexity Theory to a Hospital Complex Patient Care Program

Increasingly, complexity science concepts are informing health care design and practice. The present paper describes the implementation of early complexity science principles in a Complex Care Program with the aim of strengthening the provision of integrated care. Grounded in cybernetic network theory, Stafford Beer’s Viable Systems Model provided the guiding principles for the program’s redesign. The Viable Systems Model with its broadly applicable principles, is now the conceptual model of information management in the program. Beer’s framework has enabled a relatively small number of clinicians to coordinate care for a large cohort of patients with significant clinical complexity, and a multitude of providers, in the community setting.

Co-design with Integrated Care Teams: Establishing Information Needs

Co-design has been cited as playing a major role in the future of effective integrated care, however, there is a lack of reporting and reflection on the methods used. Information sharing is fundamental when working in integrated care, however sharing across professions, service settings and localities can be complex. Through co-design, we seek to establish a shared understanding of information needs within a newly formed integrated care team. In doing so we aim to inform future practice in the understanding of co-design.
